For this assignment, you must study these failures to address the following questions in your report:

1.  What are in common among these failures?

2.  Lessons that can be learned from these failures.

3.  What should an organization do to avoid such failures?  In other words, if you are CIO in a company, what actions would you take to ensure that such failures will not happen to an IT project that you are in charge.
