- Name and type of health care organization (stand-alone, multifacility, multicampus, community-based, etc.).
- Mission, vision, and goals
- Bed number
- Type of services provided
- Volumes
- Quality indicators
- Financial performance indicators
- Human resource challenges
- Diversity issues
- Community population statistics
